Anesthesia Evaluation Information
For optimal efficiency and tracking we are asking all licensee’s to notify the CSOMS office of the scheduled date for their evaluation. In addition, if a licensee must cancel for extenuating reasons, we ask that it be done no less that 2 weeks prior to the scheduled date. We appreciate your understanding in this matter. Per the new Dental Anesthesia Conscious Sedation Requirements effective February 2026:
“Initial office evaluations no longer require two patient cases, Per Sec. “conduct an on-site evaluation at the facility where the sedation or general anesthesia is to be performed. The department shall forward the applications of qualified practitioners who have met the requirements for the initial issuance of a permit for moderate sedation or general anesthesia as set forth in sections 20-123b-3 or 20-123b-4.”
